Newly-benefit-eligible Annual Enrollment forms

Note: This page pertains to the forms necessary for newly benefit eligible employees hired between October 22 to December 31, 2023, who wish to make changes to their 2024 benefit elections. All other employees should make their Annual Enrollment elections through myHR between October 23-November 3, 2023.

You will receive an automated email directing you to complete your 2023 enrollment in myHR (myhr.umsystem.edu). Visit the webpage titled enrolling in benefits for the first time to access the 2023 benefits guide, premium sheet, and comparison of medical plans, all of which can help you make decisions that are right for you.

As a newly benefit eligible employee, you must submit a medical insurance choice for 2023, even if that choice is to waive coverage. If you don’t, you will be defaulted to the Healthy Savings Plan, self-only coverage, and pay taxes on your insurance premiums.

If you would like to make changes to your 2024 benefit elections, which will be effective January 1, 2024, you must complete the 2024 Annual Enrollment Form and other applicable forms.

Once you’ve completed your form(s), you must submit it to the HR Service Center (umurl.us/HRSC) or your HR Generalist (umurl.us/CBR) within 31 days of your date of hire.
